Bring in this voucher to any Jewel Osco Pharmacy when obtaining a Flu Shot. Flu shots are covered by many insurance plans and are priced very competitively in the Chicagoland area.

A portion of the proceeds collected goes to TCYFL Cares and each nearby TCYFL member community to be used as youth football scholarships. Donations to each organization will vary based upon the number of shots given in the community. The top 2 organizations will receive an extra contribution.

Who can receive the flu vaccine? Anyone 14 years of age or older. (Some health related exclusions apply)

Who can use this Voucher? Any player, parent, coach, relative, or friend of the TCYFL family.

What do I do with the voucher? Just fill out your team information, bring it with you and present it to the pharmacy staff. They will take care of the rest.
